Almost two weeks ago we started talking about goals – let’s check in

Living well with a heart condition requires staying on track with your goals. SMART goals can help you create and maintain these habits by setting out how you will fulfill them. SMART stands for Specific, Measurable, Achievable, Relevant, and Time-bound.

Three tips for success

If you haven’t set yourself goals yet, now is the time. Try setting some heart-healthy SMART goals using the template below as a guide. You might choose to print your goals and keep them on your desk or fridge door as a reminder.

SMART Your goals
Specific  What do you want to achieve?Where? Who is involved?  
Measurable By how much? How will you know when you’ve achieved it?   
Achievable How are you going to do this? Is this possible?  
Relevant Why is this important to you or to your overall goal?    
Time-bound When do you plan to start this? For how long?
