Ask about recommendations to maintain a heart-healthy eating pattern, including referral to see a dietitian.
Ask about suitable recommendations for physical activity or a referral to see an exercise physiologist.
Ask about your risk factors like smoking (if you do smoke), diabetes or blood pressure and strategies to manage them.
Raise new or recurring symptoms that may need medical attention.
Ask for recommendations for either social groups or exercise groups (or both) for ongoing support.
