Time for personal reflection

Reflect on the successes of your journey

The journey from recovery to living well with heart disease can be filled with challenges, victories and opportunities for reflection.

Taking the time to reflect on your progress, successes and areas for improvement can be a valuable exercise in moving forward.

Your goals might be taking your medicines as prescribed, walking for half an hour each day, including vegetables in your lunch and dinner daily or talking to your doctor about your mental health. Even small changes, made consistently over time, make a difference.

Take this chance to review your heart health goals and make sure that they are SMART goals (specific, measurable, achievable, relevant and time-bound).

You might like to complete the questions below.

  1. What goals did you set out to achieve?
  2. Did you achieve your goals? Yes/no/partially?
  3. What do you feel like you did well (whether you achieved a goal or not)?
  4. What do you feel was more of a challenge (whether you achieved a goal or not)?
  5. How could you improve this in your next attempts at achieving this goal?
  6. What goal/s are you going to set next?
  7. How will you achieve this goal? (Think about the who, what, why, where, when and how).
