Co-designed with patients, carers and health professionals, the MyHeart MyLife program delivers information and support via digital journeys tailored to a person’s time since diagnosis, geographic location and whether they identify as a patient or carer.

For patients unable to access the digital MyHeart MyLife support program, we are developing the ‘Your next steps to recovery and living well with heart disease’ printable booklet.
