The Heart Foundation has undertaken a comprehensive process to understand the current needs of people living with coronary heart disease and their carers. This process has involved in-depth literature reviews and qualitative interviews with patients, carers and health professionals.
Informed directly by feedback from end-users, the MyHeart MyLife program delivers information and support via digital journeys tailored to a person’s time since diagnosis, geographic location and whether they identify as a carer.
We intend to tailor the MyHeart MyLife program to other heart conditions over time. In the meantime, people with other heart conditions can access the Heart Foundation website for information and support relevant to their condition.
For patients unable to access the digital MyHeart MyLife program, we have developed the ‘Your next steps to recovery and living well with heart disease’ printable booklet. This resource contains the most critical information for people newly diagnosed with coronary heart disease, available in an easily printable format.
Yes, people can join the MyHeart MyLife Facebook community without having to register for the MyHeart MyLife support program. They can do so by visiting the MyHeart MyLife Facebook community page.
