A Heart Foundation support program for people who have had a heart event, such as a heart attack, or have angina.
Health professionals are welcome to trial the support journey by registering here.
If you or a loved one have recently experienced a heart attack or angina episode, please visit our dedicated support page.
The trial health professional journey will allow you to experience the program as if you were the patient or carer. This includes everything from completing the online enrolment process, to receiving the text message and email journeys and one printed booklet sent to your home.
The only changes are that instead of an initial inpatient printed booklet you will be able to download a copy, and you will not receive our Helpline phone call.
It's free and easy to join.
