My Heart, My Life

Our team at the Heart Foundation is working hard to redesign our flagship support program for people living with coronary heart disease, and their support carers.

The new MyHeart MyLife program will be a personalised 12-week digital program that delivers relevant heart health information, expert tips, practical tools and support in the form of text messages, emails, and online content to support Australian adults living with heart disease in their journey to a healthier heart.

We are excited to announce that the new look MyHeart MyLife program will be going live in August 2024.

As a result, registrations for the current program will close on 31st July.

Stay tuned for more details and be sure to fill out our Expression of Interest (EOI) form to stay updated on the latest developments.

Thank you for your understanding and continued support.

The trial health professional journey will allow you to experience the program as if you were the patient or carer. This includes everything from completing the online enrolment process, to receiving the text message and email journeys and one printed booklet sent to your home.

The only changes are that instead of an initial inpatient printed booklet you will be able to download a copy, and you will not receive our Helpline phone call.

It's free and easy to join.
